How many days has it been since your dog was neutralized? Was his scrotal hematoma getting bigger and bigger, and is it still growing?
Castration in adult dogs has more risk of post-operative bleeding because the artery & venous flexus of testicles are much more developed(especially in large breeds such as German Shepard) than puppies, even if they were well ligated.
Since he is taking antibiotics and anti-inflammatory drugs, the probability of infection is extremely low if he does not lick the surgical site. Therefore, if the hematoma size is not large and does not get bigger, you can expect to stop bleeding and improve just by applying gentle compression to the area. However, if the hematoma is large enough like pseudo-testicle, or its size is getting bigger, it may have to be re-opened and the blood vessels should be ligated again, so you must take him to an animal hospital.

Thank you for update his state. The size of his scrotum doesn't look much different in the two pictures you posted(before hematoma & now). If the bleeding has stopped and the hematoma is improving - that is, if you can't feel the heat and the size is gradually decreasing, you can monitor it at home. Thank you.

This was before it ruptured. Warm or cold compression?

It would be better if you could post a picture of his current status. Without cold or hot packs, apply a little bunch of soft gauze or cotton and wrap an x-shaped bandage around it so that he doesn't feel uncomfortable defecation. However, some medical tapes used for humans can cause severe irritation to the dog's skin, so if you are concerned about his symptoms, it is best to go to the animal hospital where he underwent surgery and get treatment. It was like this but now much smaller since it ruptured. He was neutered on 3/18. I took him in the past Saturday and that night it ruptured. Now he is fine but still concerning since he’s my baby (18months)

The hematoma still looks quite large. You must have been very worried that your boy suddenly became like this. However, if the size of that has decreased a lot, there is not much need to visit emergency services.
Please be careful of him not to lick the surgical site or contaminate it. And it would be better if you could apply gentle compression to that area. Thank you.
